The incident occurred on January 9th, 2025, at around 5:00 PM near the intersection of SW Canal Boulevard and SW Quarry Avenue. A head-on collision involving two vehicles resulted in severe injuries to one driver, while the other occupants sustained minor or no injuries. Bradley, driving a maroon Subaru Legacy, crossed into the oncoming lane and collided with a white Toyota Tacoma driven by Samuel Palacios Arroyo. Despite immediate medical attention, Bradley succumbed to his injuries later that evening.

According to the Deschutes County Sheriff’s Office, the investigation revealed that Bradley was not wearing a seatbelt at the time of the crash.
